El impacto y el proceder médico frente a las directivas anticipadas

Resumen: El análisis del impacto y del proceder de los médicos frente a la existencia de Directivas Anticipadas presentadas por los pacientes en circunstancias especiales, no puede ser analizado sino bajo la mirada del vínculo que se establece en la Relación entre ellos. Una Directiva Anticipada (DA) nace de la voluntad del paciente, y por lo tanto lo coloca en un nivel protagónico, por ser él mismo sujeto de la acción del médico, y además por la trascendencia que esto implica en cada paciente respecto de su vida. Por tanto consideramos que ante la toma de decisiones fundamentales, cada paciente, desde una dimensión ética, deba recibir la información clara y precisa por parte de su médico, y en el mejor de los casos de todo el Equipo de profesionales que lo tratan. De este modo tendrán a su alcance las herramientas necesarias para poder evaluar la conducta a seguir, la cual deberá ser lo suficientemente pensada y reflexionada tal cual lo merece esa circunstancia. El médico, que en algunos casos puede estar en disidencia con las decisiones que toma el paciente, resigna muchas veces su propio actuar para respetar al paciente en su voluntad anticipada. Este impacto debe ser analizado por los profesionales con el fin de encontrarle un sentido a las decisiones de los pacientes; a la vez que puedan acompañar esas acciones desde lo racional y lo humano, como una de las partes constitutivas del ejercicio que a cada médico le compete.
Abstract: The impact suffered by the physicians and the actions they take in the face of Advance Directives given by the patients in special circumstances should only be analyzed based on the bond generated during the physician-patient Relationship. The Advance Directive (AD) arises from the patient´s will; this fact gives him/her a key role in the process since they are the subject of the physician´s actions and the ones who will feel in their lives the significance of such directive. Therefore, from an ethical point of view we consider that, when a patient needs to make fundamental decisions, he/she is entitled to receive clear and precise information from his/her physician and in the best scenario from the whole professional Team which is treating him/her. In that way, they will have the tools necessary to assess the paths to follow, which shall be thoroughly thought and reflected, as much as the circumstances deserve. The physician, who in some cases may be against the decisions made by the patient, gives up his own actions in order to abide by the advance will of the patient. This impact should be analyzed by the professionals in an attempt to make sense of the decisions made by the patients; while nevertheless being capable of supporting such actions in a rational and humane way, as a constitutive part of each physician´s professional performance.
