Response of the alveolar bone to the calcium hydroxide Ca (OH)2 after extraction. A preliminar study

At SAIO 2009 we claimed that the Ca (OH)2 placed in the tibiae of rats accelerated and promoted the bone regeneration. However, it is known that there are differences in the reaction between the long bone and the alveolar bone. Hence, our OBJECTIVE was to evaluate the response to the Ca (OH)2 of the alveolar bone after extraction. METHODS: male Wistar rats were used (n=15). The animals were anaesthetized and extractions of both first lower molars were performed. In the left alveoli, pure Ca (OH)2 and a piece of an absorbable sponge were placed, covering the socket. The alveoli on the right were used as controls with only the sponge. The animals were sacrificed after 7, 15 and 30 days post-extraction. Both hemimandible were dissected, fixed, demineralized, and processed to be then embedded in paraffin. Serial sections were made at the level of the mesial socket of the first molar, stained with H/E and observed microscopically. RESULTS: At 7 days in the area in contact with the Ca (OH)2, hyalinization of the bone matrix with loss of viable osteocytes and/or of osteocytic lacunae was observed. Besides, remains of this material were incorporated in that matrix. The surrounding connective tissue showed inflammatory infiltrate. After 15 days, in the peri-implant region, inflammatory infiltrate was observed, as well as nuclei of chondroid tissue surrounded by a hyaline acidophilic substance; and the bone tissue presented secreting osteoblasts in the bone marrow spaces. After 30 days the chondroid tissue was surrounded by osteons and incremental lines clearly marked, with capillary neoformation in the Havers’ ducts. CONCLUSIONS: Unlike what has been observed in the tibia, in this model the Ca (OH)2 led to inflammatory processes in the short term. In the long term, the process of the alveolar bone repair takes place.
